The basics

What the Trinity Health MyChart patient portal is

MyChart is the patient portal used by Trinity Health and by many other health systems. It is a secure website and mobile app that lets eligible patients see information from their own medical record and handle routine tasks without picking up the phone.

What you can see and do depends on your healthcare provider, your account configuration and your location. A tool your provider has not switched on will not appear in your account at all, so treat the features described on this site as what portals commonly offer rather than a promise of what yours will include.

Sign-in instructions

How to log in to MyChart Trinity Health

These are the general steps for signing in to a MyChart patient portal. Every one of them happens on the official Trinity Health website or in the official MyChart app - never on this page.

  • shield

Check the web address before you type anything. Sign-in should happen on an official Trinity Health or MyChart address, and a real sign-in page never asks for your password by email or text.

  • lock

Nothing on this guide asks for your details. No password, security code or medical record is entered here, and this site has no access to your account.

1

Open the official Trinity Health website or MyChart portal

Start from the official site or the MyChart sign-in address itself, rather than a link in an unexpected email or a sponsored search result you have not checked.

2

Find the MyChart login option

The sign-in link usually sits in the top bar or under a patients and visitors section. If your own provider's website links to MyChart, that works just as well, because it points at the same portal.

3

Enter your login credentials on the official website

Type your MyChart username and password on the official sign-in page only. No support agent and no guide ever needs to know them.

4

Complete any additional verification the portal asks for

Your provider may have a second step switched on, such as a code sent to the phone number or email address on your account. Keep those details up to date so the code reaches you.

5

Follow the on-screen instructions to open your account

Once you are in, results, appointments, messages and billing usually sit in the main menu. If something you expected is missing, your provider may not have it switched on for your account.

Why it is worth setting up

What you can usually do once you are signed in

Not every provider switches on every tool, and what you see depends on your account and your location. These are the things a MyChart account most often includes.

Registration

How to create a new MyChart account

Registration happens on the official portal, and what you need depends on your provider. You may be handed an activation code on a printed summary after a visit, or you may be able to request an account online and verify your identity instead. If your records sit with a provider that does not offer MyChart, the official site is the only place that can confirm it.

  • Visit the official Trinity Health MyChart website.
  • Select the account registration option, if it is offered for your provider.
  • Follow the identity verification instructions you are given.
  • Provide the requested information directly through the official portal.
  • Create your account credentials and complete the registration.
If you cannot get in

Forgot your username or password? Recover it on the official portal

Both a forgotten username and a forgotten password are recovered from the official sign-in page, using the links beneath the login fields. Recovery only works on the official portal, and only through the phone number or email address already registered on your account.

  • person_search

Forgotten username: use the username recovery option on the official sign-in page and answer the verification it asks for.

  • key

Forgotten password: choose the password reset link and follow the steps sent to your registered email address or phone number.

  • support_agent

Locked account or still stuck: only your provider's portal support can unlock an account or change the details held on it, so reach them through the official Trinity Health website.

Troubleshooting

Common MyChart login problems

Most sign-in problems come down to one of these. If your situation is not here, the guide for the step you are on is the better place to look.

Trinity Health patients sign in through the Trinity Health MyChart portal. If you also receive care from another health system, you may have a separate MyChart account for it, because each system runs its own portal. Your provider's own website always links to the portal that holds your records.

Yes. The official sign-in page has a recovery option for forgotten usernames. It verifies your identity using details already on your account, then confirms the username it holds. It is not something this guide, or any other third party, can look up for you.

If your provider has two-step verification switched on, the code goes to the phone number or email address on your account, so an out-of-date contact detail is the usual reason it never turns up. Check filtered and spam folders, wait a few minutes and request a new code. If it still does not arrive, ask your provider's portal support to update your details.

Locked accounts are usually released by waiting a short while, or by going through the password reset on the official portal. Only your provider's portal support can unlock an account or change the details held on it.

No. This is an independent guide, and it is not affiliated with, endorsed by or operated by Trinity Health. It explains the process and links to the official portal; no username, password or medical information is ever collected here.

Yes. The app and the website are two ways into the same account, so the same username and password work in both. If one refuses your details, the other will too, and the fix is recovery on the official portal rather than a different sign-in page.

Ready when you are

Sign in on the official Trinity Health MyChart portal

Everything here is a guide. The sign-in itself happens on the official portal, which is the only place your username and password should ever be typed. If you still cannot get in, your provider's portal support can help - this independent site cannot reset or unlock an account for you.
